I would use the Formative evaluation for the health and wellness program because the formative evaluation is an assessment during the initial stages of the implementation process and preliminary outcomes. Formative suggests that the evaluation is conducted because the program is forming or in its early stages of implementation. This may require collecting some initial but significant health outcome data. Formative evaluations also frequently entail the utilization of multiple methods, employing both quantitative methods and qualitative approaches. The qualitative approaches, such as focus groups and interviews, are often helpful in understanding problems from the attitude of either the program staff members or the program participants. The opposite requirement for a useful formative evaluation is that it is done in a timely manner, and with strong feedback and reporting to the program manager.
