CSTAR is an adolescent substance abuse program that works from a strength-based model. The population that can obtain services through CSTAR is individuals from 12 to 18 years of age. There are three phases of services through CSTAR. The first is through the residential program, where a teen can stay for up to two weeks and obtain nonmedical detox services. The client then steps down from the residential program to the second phase, the intensive day-treatment program. The intensive day-treatment program runs Monday through Friday, and clients arrive at 7:30 in the morning and stay until 5:00 p.m. During the day, they attend three groups, and they have individual and family counseling on a weekly basis alongside those who are in the residential program. After day treatment, the individual steps down to the third phase, outpat ient counseling. There, they attend counseling two nights a week; they will participate in three hours of group counseling and one hour of individual counseling each week. Ethical and Legal Considerations As noted in a conversation with John about the program, there are several ethical and legal considerations that are important for this resource. The program is voluntary, and even though the population is minors, because of the laws in [the state], parents cannot force their son or daughter into the program. The residential facility is also not a locked facility, meaning that if the client chooses to leave, the staff are not supposed to stop them. Ethically, the client’s welfare is the most important consideration, and that is why staff are trained to call the police and notify the family if the teen leaves the building. Additionally, because of [a specific law] in [name of state], when the services provided are related to substance abuse, anyone over the age of 14 must sign a release of information for their parents to have access to their medical records. The company uses both licensed mental health counselors and licensed d rug and alcohol counselors. John stated that legally, in [name of state], licensed drug and alcohol counselors can provide substance abuse counseling services but cannot provide mental health

counseling services. Any clients who go into the program and are dealing with both substance and mental health concerns are seen by a licensed mental health counselor. Application This resource is best used if the individual is dealing with substance abuse struggles. The program does offer mental health counseling and trauma-based services, but they must occur with a substance-abuse-related issue. The program works closely with local schools to provide substance abuse assessments for students who are caught bringing substances into the classroom. Because of the way the program is set up, the resource would be best used if I had an adolescent client who was dealing with substances and needed more counseling specific to substance use, particularly if they needed detoxification. Legally, I would need to remind both my client and my client’s family that this was not a locked-down residential facility and that participation in the program was voluntary, and I would encourage them to review the informed consent during the admission process. Once my client was admitte d to the program, I would want to see which consent forms would need to be completed so I could be made aware of my client’s progress in treatment, if the plan was for me to continue outpatient counseling for them later after completion of the substance abuse program. Legally, I would also need to have consent forms signed by my client’s guardians, according to my state’s laws, in order to provide any information to the residential treatment program.
